Please follow the instructions below to complete and submit the
Monsignor Lafferty Tuition Foundation, the Catherine Birzle
Scholarship and the Local Schools Tuition Assistance
Application:
How do I apply?
1. You may
apply for all three types of tuition assistance by using
one form.
2. You will
complete the application process for all students in the
family with one form
3. You may
apply either using the paper application or by applying
online.
Application forms, instructions, and
mailing envelopes may be obtained from
the school where your student(s) atten(s)
or will attend. Mail the completed
application and any necessary
documentation along with a $17.00 check
made
payable to FAIR in the envelope provided
by April 16th,
2007.
If you would like to submit your
application online, you will
need the following:
2. A credit card (Visa or
Mastercard) to complete payment
processing (your card will be
charged $17)
3. A complete copy of your 2006
Federal Income Tax Return with
all schedules and W-2’s
(must be mailed to FAIR by April
16, 2007)
4. The School Code and the
School Password for the
Monsignor Lafferty Tuition
Foundation
To begin the online
application process,
parents should go to
www.fairapp.com and
click on the “On-line
Application” link. You
can then create a user
account using your email
address and a password
that you select. Once
you have created a user
account and signed in
you will be able to
complete the FAIR
application online.
After signing in you
will be asked for a
school code and school
password. For the
Monsignor Lafferty
Tuition Foundation
please use School Code:
400 and school password:
mltf400.

am I mailing my
application to FAIR?
The
Monsignor
Lafferty
Tuition
Foundation
has
contracted
with FAIR to
process the
tuition
assistance
applications
and provide
an objective
evaluation
of each
family’s
financial
need. FAIR
is a
nationally
recognized
leader in
financial
need
evaluation
services for
Catholic and
private
schools.
Is my personal
financial
information
shared with any
organizations
other than FAIR?
All information
is held strictly
confidential.
FAIR will not
share or
disseminate this
information in
any way with any
other
organization
except for the
Monsignor
Lafferty Tuition
Foundation, the
Catherine Birzle
Educational

your student’s

Important
Notes Before
Completing
and
Submitting
Your Tuition
Assistance
Application:
1.
You
MUST
include:
a.
a copy
of your
2006
Federal
Income
Tax
Return
with
ALL
applicable
schedules,
b.
copies
of all
your
2006
W-2s
and/or
1099
forms,
c.
a check
or money
order in
the
amount
of
$17.00
payable
to
FAIR.
d.
applications
must be
postmarked
by April
16, 2007
with all
necessary
documentation.

estimated

amounts
for
2007.
3. In the dependent section of the application, please indicate the school(s) and grade(s) that your dependent(s)
will be entering in the Fall of 2007 (next school year). (Refer to Part IV of the application for a list of schools)
4. Please complete ALL items that apply to your family. If an item does NOT apply to you, leave it blank.
5. When submitting your application, please place your check and documentation inside the application.
Stapling items to the application can cause them to tear when being separated.
6. There is space provided in Part VII: Special Circumstances for you to list and explain any other expenses and/or special
circumstances that you believe would impact your eligibility for tuition assistance. Please use this space to explain anything
unique to your financial situation.
7. 7.You will receive a postcard from FAIR confirming receipt of your application. You do not need to respond to this
communication. If we have any questions or require additional documentation, we will contact you in writing.
8. Do not use photo copies of the applications packet. Additional forms may be obtained through the Diocesan
Office of Education.

Part V: Family Income
Annual Income Items:

This is from line 7 of your Form 1040 or Form 1040A.
Business Income from Self Employment
If you have income on line 12, line 17, or line 18 of your Form 1040 please put the total of these lines here.
NOTE: You must also complete Part VI: Business Income from Self Employment.
This is the total of lines 8a and 9a of Form 1040 or 1040A
Monthly Unemployment and/or Workers Compensation
If you received worker’s compensation or unemployment compensation, please put the average monthly amount you actually received in 2006 and estimate the average monthly amount you expect to receive in 2007.
Monthly Social Security and/or Pensions Received
This is from lines 16a and 20a of Form 1040 or lines 12a and 14a of Form 1040A. Total these amounts and divide by 12.

(Note: Due to a printing error, this section is marked V on the application).
The current combined balance of your checking and savings accounts
Stocks, Bonds, and Investments
List the current value of of any stocks, bonds, or other investments excluding retirement accounts
Please estimate the current market value and mortgage balance including any home equity loans or second mortgages
Value of Any Other Property and Amount Owed
Indicate the estimated value and amount owed on any other property including but not limited to cars, second homes, and rental property
Indicate the number of vehicles owned
Vehicle make, model, year, and any outstanding loan balance
The monthly amount of payments on ALL vehicles
Total annual vehicle insurance
The total amount of all credit card balances and the monthly payment made on these balances
The total balance of all outstanding student loans that are not being deferred
The total amount of any other outstanding debt you have along with a description of the nature of the debt

Include any after tax health and dental insurance premium’s, office and medication co-pays, or any other out-of-pocket medical and dental expenses

Do NOT include dependent tuitions
2006 Child Support Payments made by you
2006 Alimony paid by you to an ex-spouse
Include any other expenses you believe will impact your eligibility to receive financial aid. Please be sure to include an explanation of these expenses in Part IX: Special Circumstances
