Admissions Process & Forms
Families wishing to enroll their child(ren) at Gehlen Catholic should print out and complete the following forms and return the information to Gehlen Catholic School:
1. Admissions Form
2. Transcript Request Form (if transferring from another school)
3. Health & Safety Information
4. In Town Bussing Form (if parents will not provide transportation)
5. Internet Permission Form (needs to be signed every year; one per family)
6. FAIR application (for those requesting tuition assistance)
7. Home Language Survey
8. Dental Screening
9. Tuition Payment Information
No child will ever be denied the opportunity to attend Gehlen Catholic because of the cost of tuition. Therefore, we invite your family to schedule a confidential appointment with the Tuition Assistance Committee by calling 712.546.4181. They will work with your family to develop a plan that meets your unique needs. Tuition can be offset through the following programs:
1. Msgr. Lafferty Tuition Foundation – Working families whose annual income is at or below 300% of the poverty level may receive 25-75% off of each child’s tuition;
2. USDA Grant;
3. Local Tuition Assistance;
4. Northwest Early Childhood Iowa Application (students in Jr. Jays' Academy only); and
5. Scrip Program – Families can decrease their tuition costs just by shopping at local and national retailers!
